In accordance with the Oregon Health Authority, there are forms that each individual must complete prior us scheduling your psilocybin journey. Please confirm your information below, specify if you want to fill out the forms via text link or email and we will send you a link to fill out the initial form. Once we receive your completed form our intake and scheduling team will contact you to complete your assessment and assist with booking your journey.
